What Are Orthobiologics?
Orthobiologics are treatments that use healing substances drawn from your own body – such as blood, bone marrow, or fat – to help repair damaged tissue, reduce pain, and restore function. Because they come from you, your body recognizes them and puts them to work.

Types of Treatments
PRP - Platelet-Rich Plasma
Blood is drawn and spun in a centrifuge to concentrate healing proteins (secreted by platelets), which is then injected directly into the treatment area. These proteins and enzymes are the bodies “first-responders” that help stabilize inflammation and signal repair processes to start.
BMAC - Bone Marrow Aspirate Concentrate
A bone marrow sample – usually obtained from the pelvis – is concentrated and injected to support tissue repair and reduce inflammation. There are similar proteins as those found in PRP. EMAC also offers sell based signaling. In this sense, your body‘s own repair system is called into action when injected into the damaged area to stabilize inflammation and create a healing cascade that unfolds over several months.
mFAT / Lipoaspirate
A small fat sample is removed from your abdomen, back or hip, fragmented into tiny particles, and then injected to cushion and help repair the target area. As it turns out, our adipose, or fat, tissue also has cells that can help signal repair processes and reduce inflammation.
How are Treatments Performed?
Most procedures are performed right in the office under aseptic conditions with sterile preparation of the injection site or sometimes in a sterile surgical setting concurrent with a planned surgical procedure.
Office-based injections are typically performed with ultrasound image guidance to optimize the precise location of delivery. Surgical injections are usually done with direct access to damaged tissue.
Preparing for Treatment
We may recommend a "prehab" phase to prepare your body and improve results:
- Targeted exercises or physical therapy. Muscle activity and conditioning can prime your body’s healing potential and recover recovery.

What to Expect After Treatment
Orthobiologics are not a quick fix – they work with your body's natural healing process. This is important to no and is different than traditional treatments, which are focused on short-term pain, relief rather than long-term repair and tissue health.
Results build gradually, and usually continue to improve throughout the first 3 to 6 months and are often well worth the wait. Dr. Woolf, strongly believes that long-term health of your bones, cartilage, ligaments, tendons, and muscle should be our goal. Modification of the disease with potential for tissue healing is more important than short-term relief that does little to change the progression of arthritis or tissue damage.

Frequently Asked Questions
Will this be painful?
Most patients tolerate these procedures well. Some soreness or achiness at the treatment site is common for a few days afterward
- this is normal and actually a sign that healing is underway.
- most of the time acetaminophen (Tylenol), rest, gentle range of motion, and a short application of heat can help relieve discomfort.
Is this covered by insurance?
In general, orthobiologic treatments are currently cash-pay and are not covered by insurance or Medicare. SMOC does not submit bills to insurance
for these cash-based procedures. Our team will always provide transparent pricing before your procedure – no surprises. You will know exactly what the process and cost will be. In addition, we strive to optimize the value you get as we partner with you to improve your health.
Your personalized experience will include longer visits, extensive counseling about optimizing thr pre-and post injection factors to get the best result possible, and image-guided injection and visualization of the tissue damage. Much different than the usual 5 to 10 minute office visit. We see this as far more than just an injection.
How many treatments will I need?
This depends on your condition and how you respond. Some patients benefit from a single treatment; others may do best with a planned series over time. It also depends whether focused, shockwave therapy or other treatments are planned to improve your results or prime tissue for healing. Our goal is to determine the most appropriate treatment plan to make the most of your time and to get the best result we can.
What are the potential risks?
Common side effects include temporary pain or soreness, skin or tissue irritation at the injection site, and joint achiness. Some
patients may not experience the desired level of improvement. However, most patients, about 80%, can achieve noticeable improvement that last far longer than traditional treatments. Infection and other typical surgical risks are extremely rare.
What should I avoid before treatment?
Your provider will give specific instructions. In general, anti-inflammatory medications such as ibuprofen or naproxen are paused before and for a while after the procedure, as they can interfere with the healing response. You will be provided counseling and also guide guidelines for both before and after your treatment.
When can I return to normal activity?
This varies by treatment type and your individual condition. Your care team will provide a personalized return-to-activity plan to get you back to doing what you love – safely and sustainably. In general with ortho biologic treatments, we advise you to take it easy for the first 3 to 5 days, depending on exactly what was done. This means daily activities, but refrain from aggressive activities, including most exercise. Then you can begin walking or light activity. Return to fitness activity will follow later and then progressive stress and loading of the treated tissues will follow a timeline. This depends largely upon what was done, what was treated and the biologic type that was used. As part of your experience, guidance on activity and directed physical therapy is provided.
