When you come for your initial appointment, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required by your insurance)
  • Your medical history and current worksheet
  • List of current medications (if any)
  • Prior office notes, operative reports, and Imaging (i.e. Disks, USB drives, or printed Xray/MRI sheets containing the actual images for X-rays, MRIs, CT scans, etc.)
  • Any other relevant information to help address your orthopaedic concerns during the visit

To cancel an appointment

Please telephone the office during business hours   and allow a minimum of 24 hours notice so that we can offer your appointment time to patients on our waiting list.

Late cancellations and no-shows may be charged for the visit slot being unused.

We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patient concerns require a little more time, and these cause scheduling delays beyond our control. We hope to provide the needed attention that you and all of our patients deserve. We apologize if we keep you waiting.
